Improve Chicago Manual of Style date/publisher/place handling

Description
When publisher info is blank, formatted bibliographic references currently contain a colon and trailing space. These need to be eliminated in these cases.
Likewise, if I have a place and a year, but no publisher, I still get a comma and space between the place and year. These need to go.
If there is no date entered for an item, it should to be formatted as "n.d."
If there is no place entered for an item, it may be formatted as "n.p." though it's not clear whether this is required by CMS.
Finally, in author-date system, CMS should sort with n.d. items last, e.g.
Takats, Sean. 1999. –. 2005. –. n.d. Thompson, E.P. 1970.
